All About the Mexico Health Questionnaire
Due to the coronavirus pandemic, all visitors to the country must now obtain authorisation to enter Mexico. Mexico’s travel certificate is a mandatory document that enables safe entry into the country. If you haven’t completed the Health Questionnaire, you cannot visit.
All air passengers regardless of nationality need to fill out an online form and gain approval through the Health Questionnaire before you’re allowed to enter the country. If you’re over the age of 18 years, you should register for your own form. If you’re travelling with anyone under 18, you can complete their form on their behalf.

Mexico Health Questionnaire Application Process

You can’t complete the form until 12 hours before your flight departure, so it’s a good idea to set a reminder to complete it during the allotted time frame.
As part of your application, you will be asked to provide some personal information, including the following:
- Your flight details, including travel dates, airline, flight number and seat number.
- Your personal details, such as first and last name.
- Your e-mail address.
- Your phone number, including the area code.
- Details about what countries and cities you have visited in the last 14 days.
The application will ask you the following health-related questions:
- If you’ve been in contact with anyone with coronavirus.
- To confirm if you’ve any symptoms including frequent coughing and/or sneezing or if you have a high temperature.
- If you’ve suffered from nausea, headaches or any other acute health problem.
- If you have experienced breathing difficulties.
You’ll also be asked to provide a signature to sign off on the information and confirm that what you’ve provided is correct.
Once you’ve completed and submitted your application, you will receive a QR code via email that you’ll need to show the authorities during your journey. You don’t need to print it off, as this can be shown on an electronic device such as a mobile phone. Easy peasy!
Mexico Travel Health Questionnaire Requirements

About the QR Code
After you’ve submitted your health declaration, you will receive a QR code to show authorities while making your journey. Your personal QR code is based on the information you’ve inputted. Depending on the information, it will be either green or yellow.
You should make a note of what each colour represents, as stated below:
If your QR code is green, you can proceed through airport security and board your flight as normal.
If your code is yellow, you might need additional screening to make sure it’s safe for you to continue your journey. You’ll most likely be referred to the airport’s medical examination area for additional screening.

How Long Is My Travel Authorisation Valid For?
The QR code generated in the form is valid for 3 hours after the departure of your flight. You can use it to enter the country once, as it’s valid for a single trip to Mexico. You’ll need to complete a new questionnaire each time you travel by air.
When Can I Complete My Application for the Travel Authorisation for Mexico?
You can complete your application up to 12 hours before your flight to Mexico. Your application must be made on the same day as your flight departure, so you can’t complete it before this time.
